To get a replacement driver’s license, you will need 2 pieces of identification such as birth certificate, passport, Social Insurance Card, Credit Card, a recent T4 slip, your Current Tax Return or Tax Assessment.

A signature and birthdate needs to be in the mix when you bring your 2 pieces of ID. The best approach is to go to the registry office with as many forms of ID as you can to prove your identification and current address.
